Welcome to Integrity Community Transport LLC ("we," "our," or "us"). These Terms and Conditions ("T&Cs") govern your access to and use of our services, including non-emergency medical transportation (NEMT) services, either booked through our website, mobile application, or via phone call. By using our services, you agree to be bound by these T&Cs. If you do not agree to these terms, please do not use our services.
1. Acceptance of Terms
By accessing or using our services, you acknowledge that you have read, understood, and agree to be bound by these T&Cs. These terms constitute a legally binding agreement between you and Integrity Community Transport LLC.
2. Service Description
Integrity Community Transport LLC provides non-emergency medical transportation (NEMT) services. Our NEMT services are intended for individuals who require transportation to medical appointments or other healthcare-related destinations but do not require emergency medical assistance during transit.
3. User Responsibilities

Eligibility: You must be at least 18 years old to use our services. If you are under 18, you may only use our services under the supervision of a parent or legal guardian who agrees to be bound by these T&Cs.

Accurate Information: You agree to provide accurate and up-to-date information when booking services with us. This includes providing correct contact information, pick-up and drop-off locations, and any special requirements.

Compliance with Laws: You agree to comply with all applicable local, state, and federal laws while using our services.

Behavior: You must behave respectfully and responsibly toward our drivers and other passengers. Any disruptive, abusive, or illegal behavior may result in the termination of your ride and/or future service bans.
4. Booking and Payment

Booking Process: You can book our services through our website, mobile application, or by calling our customer service line. Bookings are subject to availability.

Payment Terms: Payment for services can be made via credit/debit card, or other accepted payment methods at the time of booking or immediately after the service is rendered. For NEMT services, billing arrangements may be made with insurance providers where applicable.

Cancellation Policy: Cancellations must be made at least [insert time] before the scheduled pick-up time to avoid a cancellation fee. Cancellation fees may apply as outlined in our booking confirmation.
5. Non-Emergency Medical Transportation (NEMT)

Medical Disclaimer: Our NEMT services are for individuals who do not require immediate medical attention. We do not provide any form of medical treatment during transportation. In case of an emergency, please call 911.

Special Needs: If you have any special needs (e.g., wheelchair accessibility), please inform us at the time of booking so that we can accommodate your requirements.
6. Limitation of Liability
Integrity Community Transport LLC will not be held liable for any delays, accidents, or incidents that occur during the provision of NEMT services, except as required by law. While we take every precaution to ensure your safety, our liability is limited to the transportation service itself.
7. Intellectual Property
All content, trademarks, and intellectual property associated with Integrity Community Transport LLC, including our website and mobile application, are the property of Integrity Community Transport LLC. Unauthorized use of our intellectual property is strictly prohibited.
8. Termination of Service
We reserve the right to terminate or suspend access to our services immediately, without prior notice or liability, for any reason, including but not limited to breach of these T&Cs.
9. Governing Law
These T&Cs are governed by and construed in accordance with the laws of the State of Illinois, without regard to its conflict of law principles. Any legal actions or proceedings arising out of or relating to these T&Cs shall be exclusively brought in the state or federal courts located in Illinois.
10. Amendments
We reserve the right to update or modify these T&Cs at any time. Any changes will be effective immediately upon posting on our website or app. Your continued use of our services after any such changes constitutes your acceptance of the new T&Cs.
